Bariatric Advantage wants to help.

Go here, print out the form, then have your Bariatric Surgical Program fill it out on your behalf and fax it to Bariatric Advantage.

Do it if you need to and don’t feel bad about needing to do so.  Your health is important.

    I had my surgeon’s office do this for me the other day, because my labs were so screwed.

    Wow…I so wish I had insurance to go to a surgeon and have this filled out for me! It really sucks when you have to choose a roof or nutrition and this is absolutely amazing that they are doing this!

    This program is a life-saver. I was unemployed for two years (during which time I had my RNY) and now only work part-time with no benefits. Getting my vitamins for free from BA is the only way I can afford them. Thanks for reposting the information. :)

    Thank you for posting this! My unemployment runs out at the beginning of next month without any further extensions and I will have to use TANF to pay rent and electric, but will not have any more money for vitamins until Summer Semester. I was worried about this!
